Lincolnshire County Council has an exciting opportunity a Community Care Officer (Level 2) position based within the Adult Care team at Lincoln County Hospital. This is an excellent opportunity to work within the hospital discharge system. The role is fixed term and runs to the end of October 2024.
About the Role

The aim of the team is to work hand in hand with NHS and third sector staff to ensure that people we support are at the very centre of their own discharge. We think creatively when planning support packages, utilising “outside of the box” thinking and solutions to ensure an individual care plan is in place that empowers the service user to be an active voice and participant. Our highest continuing aim is to provide the best service possible to service users and their families.
LCC’s Hospital Service is leading the way on the introduction of a fully integrated Discharge to Assess Hub, to support access to the right services in a more fluid way, reducing delays and delivering benefits to service users.

About the Team
We are a team of 26 Community Care Officers and Social Workers based within Lincoln County Hospital. We strive to be proactive and use a strengths-based approach, to enable our customers to return home and live as independently as possible. We put the customer at the centre of the process and advocate their wishes to achieve the best possible outcomes.
We are a supportive team and welcome the different strengths and experiences new people bring.
The role is based on site at Lincoln County Hospital and work with the people we support is face to face. You will work as part of the 7 day Adult Care rota. This equates to approximately 1 in 4 weekends worked, and these are all scheduled 3 months in advance.
